- 1) to save you a great deal of time.
Until now, you would have to either drive or take the subway to a local Department of Motor Vehicles' (DMV) office; stand in line (sometimes for as long as 40 minutes), and ask for the correct form. A round trip to a DMV office could take as much as 2 hours! It's much easier to download the forms--without ever leaving your chair!
- 2) to make it easier to fill out these forms
When possible, we translate these forms into Japanese. These translations should be used only as a guide to understanding and completing the original forms which, of course, are in English.
- Where these forms come from...
- Official government internet forms (marked by a red ball
), come directly from that govenment agency's server.
These documents will be downloaded to your computer. They have the file extension: .pdf. In order to open and view a .pdf file, you must have the application Adobe Acrobate Reader installed in your computer.
It's important to know from where these forms originate. Many times, the agencies redesign their web site and change the paths to its forms. If they do this without Anzen knowing, you will not be able to download the form; in fact, you will either get an error message or be sent to the wrong page. If this happens, you should do the following:
- If it is a New York State Department of Motor Vehicles' form (any form that deals with licenses, change of address, registration, etc.) you should go to the NY DMV home page:

In their table of contents, look for and click on "DMV FORMS". This will bring you to a very long list of available forms. Scroll down until you find the form you need and download it from that web site.If it a New York State Department ofTaxation and Finance form (example: form DTF 802, to pay sales tax on a car you just bought) you should go to their home page:
http://www.tax.state.ny.us Click on "SEARCH". When the search box appears, type in the form number you are looking for (example: DTF-802). A list might appear. If that is the case, you must read the description of each and every form that refers to DTF-802 (in English), and then choose the description which best suits your needs. Download the form from that web site.
...As you can see, a broken link can cause you to do a lot of unnecessary work. That's why, if this happens, you should:
- e-mail Anzen at mail@DriveAnzen.com and let us know that the link is broken. Just write:
"Link to form (write either the form name or form number) is broken." That's all! We'll research the error and correct it as quickly as possible.
Forms marked by a yellow ball
are the official government forms which Anzen took the liberty and time to reproduce and translate into Japanese. Important! Even though they look like an official form--they are not! They must be used only as a key to understanding the official government forms which are in English.
We do not update/redesign our forms unless there are major changes. About every 3 or 4 months we try to check if the government forms have changed; if so, we compare them to our samples. Occassionally, we will find small differences (in either design or content). If we think the difference is minor and will not confuse the reader, we do not change our form. On the other hand, if data has been moved or new, important questions have been added, we will change our form as quickly as possible to reflect these changes. If you come across a form which is very different from our sample, please e-mail and tell us which form has been changed.

were translated into Japanese. They do not look like the original government documents. Generally, these contain only instructions. Clicking on one of these documents will just bring you to another web page where you can view the translation on your browser.
- About "Adobe Acrobat Reader"
- A file that has .pdf as an extension means that it can only be read with "Adobe Acrobat Reader" (example: mv44.pdf--NYS DMV Driver License Application form). After downloading a .pdf form, some people have opened it in their word processing application. The form is unreadable. They call and tell us that something is wrong with the form. Before calling us, please make sure you have "Adobe Acrobat Reader" installed on your hard disk!
When you click on the "Get Acrobat Reader" icon below, it will take you to the Adobe home page. There, choose the correct Acrobat Reader for your platform (e.g., Windows, Mac, etc.). The application will be downloaded to your computer. Depending upon your modem speed, downloading can take between 30 to 60 minutes! Afterwards, you will need to install the application, and restart your computer. Then, when you double-click on a .pdf file, it will open using "Acrobat Reader."
